Exercise Questions | Exercise or surgery on rotator cuff?

I have always been athletic and have kept pretty fit. However, I sustained a shoulder separation while playing softball in my 40's and also a shoulder separation playing touch football whithin a couple years of that injury, all on the same side. Now 20 years later, I cannot perform a military press from behind the neck because of left shoulder rotator cuff pain. A palms facing in chin is possible but hurts also. My left clavicle sticks out compared to the right.
Should I opt for surgery?
